Hansa Medical Groupe (HMG) is a concierge-style medical practice providing on-site care specifically for residents in senior care environments. Working closely with the Amazing Grace staff, HMG keeps patients healthier and happier, ensuring high quality medical care and improving outcomes.
Memory care and assisted living residents at Amazing Grace don’t have to worry about seeing a new face every week. The 5-star rated HMG’s Chief Medical Officer, Dr. Chirag Patel, builds his medical team—including doctors, specialists, nurse practitioners and RN’s—with a commitment to excellence. Consistent members of Dr. Patel’s team will conduct weekly visits and monthly wellness checks on site for our residents’ convenience. Providing a dedicated team to the Amazing Grace community allows the providers to build meaningful relationships with residents and truly understand their personal needs. Specialists are also available to visit as needed for those with chronic or severe health issues. Through HMG, Amazing Grace residents will enjoy benefits such as:
